Thanks to its direct connection with the ultrasound device, ESI provides planning-driven visualization of needle and optical fiber paths, facilitating insertion through dedicated guides aligned with the ultrasound plane. With the integrated software, the physician can simulate insertion paths, choose the most effective fiber arrangement, and accurately estimate the treated area, taking into account safety margins relative to nearby critical structures — essential for both benign and malignant lesions. Pre-treatment planning for improved lesion targeting
The pre-treatment planning software supports the operator in determining the most appropriate treatment parameters for each specific clinical case and treatment objective: achieving Laser-Induced Cytoreduction (LICR) in the treatment of benign lesions or complete necrosis in the treatment of malignant lesions.
Applications of Laser-Induced Cytoreduction (LICR) – Benign lesions
The goal of the treatment is volumetric reduction of benign lesions, such as benign thyroid nodules (BTN) or benign prostatic hyperplasia (BPH). For these applications, the ESI software allows the operator to set treatment parameters (applicator insertion angle, number and spacing of applicators, pullback, etc.), while displaying in real time the needle insertion trajectories and a closed perimeter representing the safety distances required to protect non-target anatomical structures from thermal effects. Correct treatment planning is achieved by maximizing the displayed perimeter without extending beyond the lesion or including surrounding anatomical structures that are not intended to be treated.
Applications of complete necrosis – Malignant lesions
The objective of the treatment is complete tissue necrosis. For these applications, the ESI software adds to the functionalities used for cytoreduction planning the visualization of an additional perimeter, useful for lesion centering and introducer needle placement. Proper treatment planning is achieved by including both the lesion and the desired additional ablation area (ablative margin) within this perimeter, while still respecting safety distances from critical anatomical structures. If any of these conditions are not met, the operator should consider repositioning the needles.
Planning verification
This function allows the operator to verify the positioning of the introducer needles within the target tissue. It is a valuable tool to confirm the actual placement of the applicators before starting the ablation procedure.
Real-time monitoring
The ultrasound image provided by the monitoring system is replicated in real time on the ESI interface, allowing continuous real-time monitoring of the treatment.
Interface with matrix needle guide
The dedicated software for the treatment of benign prostatic hyperplasia and prostate cancer displays on screen the needle insertion points made available through the use of matrix needle guides mounted on biplanar ultrasound probes. The operator can thus place multiple applicators within the prostatic lobes, easily reaching the optimal positions required to perform the thermal ablation treatment.
Remote assistance
By activating the remote assistance mode, the operator can share the EchoLaser Smart Interface screen in real time to discuss and agree on treatment planning with colleagues experienced in the procedure during the learning phase, or to request clinical support for complex cases. This mode can also be used to receive technical assistance from Elesta product specialists.
